This is my first KDM-Theme so don't expect too much.. If I made a mistake - just tell me, I'll correct it.. :)
Use KDM-Theme (http://www.kde-look.org/content/show.php?content=34690) to install this theme via KCM.. I've added "login-fields" for many distributions. If you want to change the icon in your login-field (default: dont't panic - thumb), you'll have to unpack this package, rename "login_YOURDESIREDDISTRIBUTION.png" to "login.png" and repack the whole directory...
